docs: drop redundant 'This fork:' self-link in README header
The line pointed back to phobicdotno/Codesys-MCP-SP21-plus, which is the URL the visitor is already on when they're reading this on GitHub. Upstream + npm + maintainer lines give enough provenance for visitors arriving from npmjs.com or via search.
